OPTIMIZING MEDICATION
A medication review is a structured evaluation of a patient‘s medicines with the aim of optimising medicines use and improving health outcomes. RECIPE has developed a website where you can find tools to help you with performing a medication review. For examples, it contains an easy tool that tells you when to prescribe a proton pump inhibitor prophylactically in case of anticoagulants. Additionally, the website contains short and readable summaries of frequently used guidelines (e.g. thrombosis prophylaxis).
